Erectile Dysfunction (ED)

Erectile dysfunction is one of the most common concerns men bring to Noumena Medical — and one of the most commonly dismissed or self-managed for years before a proper consultation happens.

ED is also one of the earliest warning signs of cardiovascular and metabolic disease. Treating the erection alone — with a pill, a device, an injection — without examining what is driving the problem is a missed opportunity for the patient’s broader health. Our approach begins with a proper history, clinical examination, and where appropriate, screening for underlying causes.

Premature Ejaculation (PE)

PE is the most common male sexual concern and one of the least openly discussed. It is also one of the most treatable — outcomes are generally good with a plan tailored to the type of PE the patient has.

Management ranges from behavioural techniques to topical and oral medications. The type of PE — lifelong or acquired, consistent or situational — guides the approach. Dr Matthew Tan takes a full history before recommending treatment, because the same symptom can have very different causes and respond to very different interventions.

Adult Circumcision

Adult circumcision is one of the most common procedures performed by Dr Matthew Tan. Men come for a range of reasons — recurrent infection or inflammation, phimosis, a tight frenulum, religious or cultural reasons, hygiene preference or aesthetic. All are valid, and all are discussed without assumption.

Dr Tan performs adult circumcision using a traditional sutured technique rather than ring devices. The reason is straightforward: sutured circumcision allows precise control over how much skin is removed, where the line sits, and how the edges are brought together. Dr Tan’s background in Paediatric Surgery at National University Hospital, where he developed refined stitching technique, informs the care and precision he brings to this work. The result is more controllable.

Foreskin Surgery

Preputioplasty (Foreskin Widening)

For men with phimosis — a tight foreskin — who wish to keep their foreskin rather than proceed to circumcision, preputioplasty is an alternative. The procedure widens the foreskin opening with small incisions rather than removing tissue. It is suitable for selected cases and Dr Matthew Tan discusses whether it is the right option during consultation.

Frenulectomy / Frenuloplasty

A short or tight frenulum can cause discomfort during erection or intercourse and can be the cause of premature ejaculation in some men. Surgical release is a brief outpatient procedure and recovery is generally straightforward.

Scar Revision

Scarring from previous circumcision or other genital surgery — whether functional or aesthetic — can be improved. Scars can not only be unsightly but because they contract over time, they can also cause pain. Suitability depends on the nature of the scar and what you are hoping to achieve.

An honest assessment of what is achievable is part of the consultation.

Penile Aesthetics

If something about the appearance of the penis doesn’t look or feel right to you — whether following surgery, scarring, or simply a concern you’ve carried for a while — it is worth raising in consultation. These are sensitive concerns and they are treated as such.

Male Fertility

Male factors contribute to roughly half of all fertility difficulties — a fact that is often overlooked when couples are investigating why conception isn’t happening.

What causes erectile dysfunction, and is it always a sign of something more serious?

There are 4 other causes of ED and they are not limited to cardiovascular – hormonal, vascular, neurogenic and psychogenic. Each etiology comes with its distinct symptoms and prognoses. These causes are also not mutually exclusive and men can have more than just one cause. Together, this makes the management of it more challenging.

Is premature ejaculation treatable?

Yes, and outcomes are generally good. The type of PE matters for the treatment plan — lifelong PE responds differently from PE that has developed over time, and treatment ranges from behavioural techniques to topical agents to oral medication. The conversation is the most important part of the consultation because the same symptom can have very different causes.

What’s the difference between circumcision, preputioplasty, and frenulectomy — how do I know which I need?

Circumcision removes the foreskin entirely, but without removing the frenulum. Preputioplasty widens a foreskin that is tight but does not need to be removed — it is appropriate for phimosis in men who wish to keep their foreskin. Frenulectomy addresses a short or tight frenulum specifically, without affecting the foreskin itself. The right procedure depends on your anatomy and your concern. An honest assessment of which is appropriate — and whether any procedure is actually needed — is part of the consultation.

What are my options for ED beyond medication?

Oral medications are effective for many men and remain the first-line treatment for most. Low-intensity shockwave therapy is an option for vasculogenic ED in appropriate candidates — it works by stimulating new blood vessel growth rather than temporarily increasing blood flow. Lifestyle and metabolic optimisation — weight, exercise, sleep, hormonal health — can meaningfully improve erectile function in men where these are contributing factors. The right combination depends on the cause.
